Student with Disabilities
- We are committed to ensuring the rights and promoting the dignity, self-awareness
and self-advocacy of students with disabilities throughout the university.

DeVry Student Services offers a variety of support services to address the
individual needs of students with documented permanent or temporary disabilities.
Students served include those with mobility limitations, learning disabilities,
hearing and visual impairments, attention deficit disorders, psychological disabilities,
and other types of disabilities such as acquired brain injuries, chronic or
intermittent health problems, seizure disorders, cardiac disorders, multiple
sclerosis, pulmonary disorders, and carpal tunnel syndrome.
Accommodations and Support Services
Accommodations and support services are designed to integrate students into
the general campus programs and activities. Students eligible for these services
need to plan ahead.
Admission Support
Applicants with disabilities who are seeking special admissions consideration
should contact the Student Services department and speak with a disability advisor
regarding their circumstances.
Mobility Orientation
A Student Services staff member will conduct an orientation for students with
mobility challenges. Best routes to the classrooms will be explored along with
accessible campus facilities. To arrange for a mobility orientation, please

Disability Management Counseling
Disability management counseling is available to students who are registered
with the Student Services. These meetings are confidential, and focus on assisting
and supporting students in dealing with disability-related issues as they impact
their role as students at DeVry.
Parking Fee Waiver
A DeVry student with a verified disability may be eligible for a waiver of parking
fees. To qualify the student must
1) have documentation of disability verified by the Student Services' Disability
Advisor.
2) possess a distinguishing license plate or placard issued by the Department
of Motor Vehicles.
3) demonstrate financial need according to the need analysis procedures for
campus- based financial aid.
4) attend DeVry either full or part time.
5) not receive any other assistance for parking fees from the Department of
Vocational Rehabilitation or other financial aid sources.
Please stop by the Student Services and speak with the Disability Advisor about
receiving a Waiver for Campus Parking Fee. A new form must be completed for
every quarter a fee waiver is requested.
How to Use Our Services
Registering with Student Services - Intake Appointment
"Intake" refers to the process through which a student becomes registered
with Student Services for accommodations. The process begins with a one-on-one
meeting with an advisor. This meeting provides an opportunity for the student
to share information about his/her disability. The advisor will review the documentation
and determine with the student the appropriate accommodations and services at
DeVry, Fremont. At the conclusion of the meeting, the student will be provided
with the Special Accommodation Request form (SAR), which will be used to initiate
the process of determining and approving services.
For students new to DeVry, Fremont, an intake appointment should be scheduled
as soon as notification of acceptance to the University has been received. Documentation
of each disability should be brought to the intake appointment. If there are
difficulties in providing the documentation at the point of intake, we will
allow four weeks for its submission. To arrange for an intake appointment, please

Documentation Procedures and Verification Guidelines
The student is responsible for submitting valid documentation when registering
with the DeVry-Student Services. This documentation is required by the University
and serves to assist us in determining the appropriate accommodations. The following
forms are used to document disability. Please call Student Services at (510)

- Verification of Learning Disabilities (LD)
- Verification of Attention Deficit/Hyperactivity Disorder (ADHD)
- Verification of Psychological Disabilities
- Verification of Other Disabilities
Guidelines for Use of Support Services
Student Services offers a wide variety of support services and accommodations
to students registered with our office. Students are expected to take an active
role in asking for and arranging their own support services. The Disability
Advisor oversees the implementation, facilitation and evaluation of the services
and is available to assist students in making these arrangements. Following
are the general guidelines for using support services.
- Register with Student Services: Students requesting support services must
have completed an intake and must have a current SAR form on file.
- Fill out a SAR: Each semester, a completed SAR form must be on file before
the services will be rendered.
- Plan ahead: While Student Services responds as quickly as possible, due to
the complexity and high volume of requests, we must ask for 30-calendar days
from the date of the request to arrange for the service.
- Act quickly: We respond on a first-come, first-served basis. Late requests
may take additional time to arrange and/or a second-choice service may be provided
in place of the initial request.
- Be responsible: Ultimately, students are responsible for ensuring that support
services and accommodations meet their needs. If a service is not appropriate
or is lacking, students must notify Student Services as soon as possible so
changes can be made.
- Adjusting services: Any changes in needs or requests for additional services
must be supported by documentation, evaluated by a Student Services advisor
and an approved SAR form must be on file, before the additional service is provided.
Meeting with Instructors
We strongly encourage students to schedule a meeting with their instructors
early in the quarter to review accommodation needs as indicated on their approved
Accommodation letter.
